If you are seeing stubborn weight gain during perimenopause, especially around your middle, and the old "eat less, move more" advice is failing you, please know this: It’s not your fault, and your body is not broken.

Do you feel frustrated when it comes to midlife weight management?
At the Cordillera Wellness Collective, we believe in sustainable health, not quick fixes. That means moving beyond the scale and using a targeted functional medicine approach to address the root causes of weight changes in this new season of life.
Here is the mindset shift I encourage every patient to make for true longevity.
1. Shift Your Focus: The Metrics That Truly Matter
In my practice, we prioritize how you FEEL over the digits on the scale. While the number can be a good starting point, it shouldn't be something we check every day, especially if you have a bad relationship with it. In that case, throw it out!
The truth is, weight loss for midlife women is unlocked by internal metrics that truly reflect your metabolic health. The things we focus on MORE than the scale are:
How are your clothes fitting?
How are you lifting in the gym (moving more weight and getting stronger)?
How is your sleep, digestion, and energy levels throughout the day?
We will clinically check markers like insulin and inflammatory markers when we start and again after 4–6 months to track concrete improvement, but your daily quality of life is the primary gauge of success.
2. The Hormonal Truth: Why Old Advice Fails in Perimenopause
I know how frustrating it is when you try to calorie-restrict and ramp up cardio, only to see the scale increase. This is because perimenopause hormonal changes make that old "eat less, move more" advice not just ineffective, but often counterproductive.
The key insight is that estrogen is anti-inflammatory and makes our cells insulin-sensitive. When estrogen starts dropping, you get increased inflammation, and your cells can become insulin-resistant. Plus, poor sleep patterns due to hormone shifts make ghrelin (the hunger hormone) increase, and leptin (the satiety hormone) decrease.
If we continue to aggressively "eat less" and "move more," we risk putting ourselves further into the ground.
Our focus must shift:
Prioritize nutrition by eating more whole foods and focusing heavily on protein.
Move with intention, which means picking up weights instead of running on the treadmill for hours.
3. The Hidden Root Cause: Your Gut Health
Beyond hormones, diet, and exercise, I often uncover a hidden root cause of stubborn midlife weight gain using our functional medicine assessments: GUT HEALTH.
If your microbiome is disrupted or you have low stomach acid, it's much harder to lose weight. Fewer good bacteria means fewer SCFA (short-chain fatty acids), which play a role in weight loss, inflammation, and protecting the gut lining.
When we focus on getting the gut working and optimizing hormones, patients finally start to see the weight change. This integrated, evidence-based practice is what sets us apart.
4. The Non-Dietary Habit for Long-Term Success
To achieve true Prevention & Longevity, weight loss must be sustainable. It requires healing that relationship with yourself, with movement, and with food. We need to avoid those old habits of juice cleanses and starving ourselves. Realizing that slow, sustainable weight loss (0.5–2 pounds a week) protects your muscles and bones and sets you up for long-term success.
My favorite 'slow and steady' habit for rebuilding metabolic health? Adding in walks.
A simple 5-minute walk after every meal adds up to 15 minutes of movement a day.
This simple act helps with better digestion and blood sugar regulation.
